Job Description Job Description PRN Days LPN - Hospice Overview:
This role focuses on delivering skilled nursing care to hospice
patients in their residences, operating under the supervision of a
Registered Nurse. Responsibilities include assessing, planning,
implementing, and evaluating the care needs of patients. Applicants
should have completed an accredited practical nursing program,
possess a valid LPN license, and have a minimum of one year of
nursing experience. The position demands physical endurance, a
valid driver's license, and may necessitate additional
certifications such as IV certification and CPR. Discover your

providing skilled nursing services to patients in their homes,
guided by a Registered Nurse. You will assess, plan, implement, and
evaluate the care and needs of patients. Position Details: Minimum
Qualifications: Licensure: Must be a graduate of an accredited
practical nursing program; equivalent educational experience and
successful licensure may also be considered. It is essential to
obtain and maintain a current LPN license in the state where
nursing duties are performed, adhering to all State Board of
Nursing regulations. A valid driver's license is required.
Experience: At least 1 year of nursing experience.
Certification(s): CPR (Cardiopulmonary Resuscitation) certification
is mandatory. Preferred Qualifications: Certification(s): Note:
Depending on the specific position or unit, one or more of the
following certifications may be required, or certification must be
obtained within a specified timeframe: • IV Certification within
one year of hire • Additional unit-specific certifications as
necessary. Physical Requirements: • The role necessitates the
ability to regularly push, pull, and/or lift up to 50 lbs. •
Prolonged standing and walking are required throughout each shift.
• The position involves gripping, reaching, bending, kneeling,
