Under the guidance of the primary care operations team, the Primary
Care Nurse collaborates closely with our Primary Care Providers to
deliver safe, timely, effective, efficient, equitable, and
patient-centered care in a structured manner. This role involves
partnering with patients, healthcare providers, and other clinical
team members to enhance quality outcomes for an assigned patient
panel. Responsibilities will be executed in alignment with our
mission, values, and service standards. Position Overview: Primary
Care Clinic Location: 12700 Southfork Road St. Louis, MO 63128 Work
Schedule: Monday - Friday No weekends or holidays! Completion of
three months of on-site training is required before transitioning
to a hybrid work-from-home schedule; availability for monthly
clinic meetings and 1-2 on-site visits per week is also necessary.
The Primary Care Nurse position is a newly established role within
our clinics. In this capacity, the nurse will closely collaborate
with Providers and manage triage calls and messages for the clinic.
After training, there will be an opportunity to work in a hybrid
model. Qualifications for LPN: Education: Diploma in Practical
Nursing Licensure: Current LPN license Experience: Preferred 2-3
years of relevant experience Qualifications for RN: Education:
Associate of Applied Science in Registered Nursing, graduation from
a registered nursing program, a bachelor’s degree in nursing (BSN),
or 4 years of equivalent experience in lieu of a bachelor’s degree
Licensure: Current RN license Experience: Preferred 2-3 years of
